Your I-693 form can be completed within 1 week or less
All necessary standard adult vaccines are available *

Please bring a photo ID
You do not need to bring the I-693 form
Expect to perform the mandatory/CDC required testing during your visit, which includes the new tuberculosis blood test (Quantiferon gold), syphilis and gonorrhea, for all applicants above the age of 15 years
Please bring any vaccination records if available. Any vaccinations administered by our office are at an additional cost
Bring someone to translate
Cash payments are only accepted, there are ATMs in close proximity to our office

* Varicella vaccines are not included and not necessary if there is a history of chicken pox
