ABOUT OUR PROVIDERS
John M. Ray, Jr., M.D.
Dr. Ray is a native of Fayetteville, North Carolina. He was educated in the Cumberland County School System and graduated from E.E. Smith High School. He earned a B.S. in biology from North Carolina Central University. He received his M.D. degree from Meharry Medical College and from there completed his residency training at the Duke/Southern Regional Area Health Education Center.
Dr. Ray is board certified in Family Medicine and has been practicing in Fayetteville since 2002. He is a strong believer in prevention and in working together with patients to take care of all of their healthcare needs. He is the father of two children.
Dr. Leah Yearwood
After graduating from medical school, Dr. Leah Yearwood did her 3 years of internal medicine residency training at SUNY Downstate, Brooklyn New York. In 2014 she took up a job as a hospitalist in Fayetteville, NC at the Cape Fear Valley Hospital, while serving part-time in the community as a primary care doctor. She joined the Pinnacle Family Care practice full time in September 2018 and brings with her 12 years of postgraduate experience.
Dr. Yearwood is passionate about getting to know the patient as a person and is known for her caring nature while offering the latest in medical technology and knowledge in an age where this is always changing.
In her spare time, Dr. Yearwood enjoys spending time with her husband and children, outdoor activities, and travel.

All necessary standard adult vaccines are available *
* Varicella vaccines are not included and not necessary if there is a history of chicken pox
Please bring a photo ID
You do not need to bring the I-693 form
Expect to perform the mandatory/CDC required testing during your visit, which includes the new tuberculosis blood test (Quantiferon gold), syphilis and gonorrhea, for all applicants above the age of 15 years
Please bring any vaccination records if available. Any vaccinations administered by our office are at an additional cost
Bring someone to translate
Cash payments are only accepted, there are ATMs in close proximity to our office

What to Expect at Your Yearly Physical Exam
Check your portal account to see if there are health reminders or questionnaires that need to be completed.
During the visit, you will receive a comprehensive examination.
We ask that you come in (2) two days before your visit for labs with Lab Corp or (2) weeks before your visit for lab draws. If you are unable to come ahead of time, be prepared to have labs drawn at your visit. If your labs were drawn before your physical you will receive your results at your physical. When labs are drawn at your physical, results will be given in 72 hours.
Physical Exams are for non-problem visits. If you have a problem based on insurance guidelines you will be charged a co-pay.
*Those getting fasted labs should do the following:
Fasted labs scheduled in the morning should not have breakfast.
Fasted labs scheduled in the afternoon may have breakfast, but should not have lunch.
*Take all medications as prescribed unless told by the provider not to.
What to Expect at Your Annual Wellness Exam or Welcome to Medicare Physical Exam
This is not a traditional visit you will not get a head to toe exam.
These exams are for screening and patient information only.
Healthcare power of attorney and code status will be discussed.
A plan for your ongoing health will be given.
Minimal labs will be done at this visit.
